Preventive Services assess family needs and address their concerns by using the Solution Based Casework model to co-create a plan for well-being and stability. The ultimate goal is to keep families together by providing supportive services.

WHAT WE DO

Family service centers provide free community-based services to help families in crisis and to ensure their children remain safe in their homes. We provide an assessment of needs, counseling, parenting-skills workshops, anger management training, assistance with obtaining benefits, referrals for additional resources, advocacy for educational needs, and help with navigating systems and resources for legal, medical, employment, housing and immigration.

The Solution Based Casework Model ensures that families are able to provide input and come up with an action plan with our staff. Success is more likely when families feel like the agreed to plan was developed with them in mind. We also find success by being flexible to those we support. Wide-ranging hours of operation combined with the ability to accommodate office, home and virtual visits keeps families engaged with Heartshare St. Vincent's Family Services from initial meeting to successful case closure. Our ability to provide either bilingual staff or translation services allows us to accommodate more New Yorkers in need, including, but not limited to undocumented families and/or those without insurance.

WHY WE ARE HERE

We want youth to avoid entering the foster care system, and we want families to remain whole — and thrive. Preventive Services are a crucial piece is achieving these goals. By providing a trauma-informed approach we are able to engage families and develop a service plan that families then buy into. Because of our services we see a clear reduction of reports regarding allegations of abuse and neglect.

We have dedicated and motivated staff who provide much needed support to the families we serve. We assist families with finding resources to address all their needs and monitor that they follow up with the referrals provided.

Our three models include Family Support located in East Flatbush, Family Treatment and Rehabilitation serving all of Brooklyn residents, and Specialized Medical program serving all of New York City residents.

Family Treatment & Rehabilitation

Mental Health & Substance Abuse Support

Family Treatment and Rehabilitation is a more intensive prevention program, serving families with children under 18 who have mental health and/or substance abuse concerns. It operates across all of Brooklyn. These supports and therapeutic referrals help stabilize families in or entering crisis, and prevent children from being placed outside their home.

Evidence-based Modalities for Families

Preventive service utilizing evidence-based modalities to keep families whole and avoid separation. The Family Support Prevention program is located in East Flatbush, Brooklyn and specifically services families in community districts 15, 17, and 18. Neighborhoods and surrounding areas include: Sheepshead Bay, Gerritsen Beach, East Flatbush, Flatlands, and Canarsie.

Special Medical Prevention

Supports Focused on the Medically Fragile

Special Medical Preventive Services help parents learn how to care for very vulnerable children with multiple medical disabilities or mental health conditions. Heartshare developed the program in response to those fragile children with serious health conditions who have unmet medical needs.

Families with children under age 18 and living with a medical condition that impacts the family and places the children at risk are eligible for services if living in any of the boroughs in New York City. Multiple languages such as, English, Spanish, Creole and Urdu, are spoken by staff to better accommodate family needs. Our professionally trained and caring staff works with families to develop a plan that best meets your family’s individual needs.
