Our Foster Care programs match youth in care with temporary, trained caregivers, often family members, when the parent is unable to care for them safely.

WHAT WE DO

While youth are in care, our case planners work alongside the youth's parent(s) — connecting them with support and services — so that the youth can return home safely. While living with a foster family, youths attend school, and receive medical, dental, and therapeutic care.

Heartshare's St. Vincent’s Family Services suite of programming is interconnected. We have worked to build a strong youth development program, so that while youth are in care with us, they are able to access tutoring, a variety of workshops, internships, and extra-curricular activities that build a strong foundation for their life after they've achieved permanency.  We have also developed on-site mental health and pediatric medical clinics, so that we can serve the clinical needs of our youth and families under one roof, making our services coordinated and easy to access.

Our Home Finding Program works with folks in our communities who care for youth in need of our support. We recruit, train, and license foster families who are interested in opening their homes and hearts to youth in foster care. Our foster families come to us for many reasons, but all receive our strong support. We are one of only five agencies in NYC who also carry special designation as a Special Medical Foster Care program. Our foster families can receive special training to care for youths with special medical needs.  Heartshare's Nuevo Camino program matches unaccompanied minors from around the world with transitional foster families, while they await connection to their Sponsor Family in the United States. We also operate Healing Homes across Brooklyn and Queens, which provide a supportive and nurturing environment for youth aged 12 to 21 in foster care.

We believe that every youth deserves a safe, happy, and permanent family. Heartshare St. Vincent's Family Services partners with parents to support their parenting goals, through referrals for therapeutic services, housing support, Health Home care coordination, high quality visitation, and more.

While youth are in foster care, Heartshare foster families ensure that youth maintain connection to their families, while providing a safe, caring, and stable home. If youth cannot return to their parent, Heartshare works to join youth with their loving family forever through Kinship Guardianship and Adoption.

Our programs ensure that children in foster care experience a seamless transition to a safe, stable and loving home. The ultimate goal is to achieve permanency, either through family reunification, kinship guardianship, or adoption. We provide intensive support to older adolescents to prepare them for transition to adulthood. All of our foster care programs include high levels of support.

Enhanced Family Foster Care

The Enhanced Family Foster Care (EFFC) program is an innovative model of care, in which youth and families can access a broad range of therapeutic supports and services without needing to transition between foster homes or foster care programs. EFFC recognizes that all children placed in foster care have experienced some level of trauma and that their needs change over time.  EFFC increases placement stability for youth by supporting the formation of a trusting and secure relationship between the child and caregiver.

Families in Heartshare's EFFC Program receive enhanced case management as well as a full array of services and supports to help them thrive as they work towards permanency.

Specialized Family Foster Care

Deeper Level of Care and Support

Heartshare is one of five foster care agencies in New York City that provides Special Family Foster Care (SFFC) for children with significant medical needs and/or developmental disabilities.

Youth in Heartshare's SFFC program are assigned to both a specialized case planner and a nurse who visits the family, attends medical appointments, and ensures that the youth is achieving health goals and following their treatment plan.  Heartshare's Treatment Team, including the nurse, case planner, care coordinator/health home coordinator, and clinicians coordinate all healthcare services as well as provide ongoing pediatric services, assessment, and health education. All SFFC foster families receive specialized training and certification to meet the needs of the children.

Nuevo Camino

Temporary Placement for Unaccompanied Minors

Every day, young migrant children arrive at our Southern border exhausted, afraid, and alone. They are among the thousands who travel to unimaginable lengths to flee violence, poverty, or natural disasters in their home countries with the hope of finding safety and security in a new place. Just as our Foster Care Program ensures that children in foster care experience a seamless transition to a safe, stable, and loving home, Nuevo Camino will work to provide temporary foster care families for unaccompanied minors ages 5 to 17 arriving in the United States without their parents in hopes of a better life. Foster parents will provide this transition home, until a more permanent home with a trusted family member can be confirmed.

Healing Homes

A Safe Space to Heal & Thrive

Heartshare supports four Healing Homes, which are Qualified Residential Treatment Program (QRTP) with a home-like environment providing a safe and nurturing atmosphere for children and young adults between the ages of 12 and 21, while they are in the foster care system. These homes provide support for young people in foster care to gain independent living skills, attend school, participate in community activities, and receive personal, academic, and career counseling.

Supervised Independent Living Program (SILP)

Transitional Living for Youth Moving Toward Independence

Short-term (12-18 months) transitional programs for youth between the ages of 18 and 21 who are preparing to leave foster care with the permanency planning goal. This is a program designed for youth who are able to live on their own in the community. This is an important step towards a path of full independence.
